The Annual Fee
The Comprehensive Preventive Medical Examination,
Personal Wellness Plan and all of the amenities
of a small practice are included in the annual
fee.
- The annual fee is $1800 per year, or $150 per
month.
- Additional family members, ages 16 to 25,
may participate for $600 per year or $50 per
month.
The annual fee is not in lieu of insurance. You
will still need health insurance to cover other
visits not included in the annual fee. Also, your
existing coverage will be used for care you need
outside of my office with specialists, hospitals,
labs, x-rays, or pharmacies. Your health plan may
reimburse a portion of the fee. Please check with
your plan administrator, employer, or insurance
agent for details.
How to Join
If you would like to enjoy the benefits of my
new intimate practice, please join by reading through
the Participation Agreement and completing the
Authorization Form. If you don’t already
have the forms, they are available from my office
at (805) 682-3771. Also, please call this number
